When to use

Chest US is targeted for pleural effusion, diaphragmatic mobility, thoracentesis marking and bedside assessment (POCUS) in critically ill patients. High sensitivity for small effusions (≤ 50 mL) and pneumothorax — superior to plain radiography.

What to evaluate

  • Pleural space: presence/absence of fluid, estimated volume, septations, content (anechoic, echogenic, debris).
  • Diaphragm: respiratory mobility, thickness, paralysis signs.
  • Superficial lung parenchyma: consolidation, air bronchograms, B-lines (interstitial syndrome).
  • Barcode/'stratosphere sign' on M-mode → suspected pneumothorax.
