When to use
Thyroid US is the modality of choice for nodules detected clinically or as incidentalomas. It characterizes solid/cystic pattern, vascularity, margins, halo, microcalcifications, size and stratifies risk via TI-RADS — guiding FNA indication.
What to evaluate
- Thyroid volume (each lobe: V = c × w × h × 0.479; normal: 7–12 mL ♀ / 8–15 mL ♂).
- Per nodule: composition (solid/mixed/cystic), echogenicity, shape, margins, echogenic foci, 3-plane dimensions.
- Location (third, lobe), Doppler vascularity, peripheral halo.
- Cervical lymphadenopathy (levels II–VI): shape, hilum, intranodal microcalcifications.
- TI-RADS category per nodule + FNA indication if applicable.

Istmo: cm. Volume: cc.
Volume total da glândula: cc.
Não há evidências de nódulos na topografia das paratireoides.
Estruturas vasculares cervicais preservadas. Ausência de linfonodomegalias cervicais.
CONCLUSÃO: Ultrassonografia da tireoide dentro dos padrões da normalidade.

Frequently asked
- Which TI-RADS — ACR or EU?
- Most Brazilian services use ACR TI-RADS (point system). EU-TIRADS is the European alternative. Standardize one system per service to avoid report variability.
- When to indicate FNA?
- By ACR TI-RADS: TR3 ≥ 2.5 cm, TR4 ≥ 1.5 cm, TR5 ≥ 1.0 cm. Suspicious lymphadenopathy or relevant family/clinical history shifts the threshold.
- How to measure thyroid volume?
- Volume of each lobe: length × width × thickness × 0.479. Sum lobes. Approximate normal: 7–12 mL ♀ and 8–15 mL ♂. Diffuse hyperplasia > 18 mL.
